Eligibility Requirements:
- At this time, all appearances must be virtual
- All requests must be submitted through the below online form with the appropriate fields completed
- All requests must be submitted 8-12 weeks in advance; the Storm will provide notice on whether or not we will be able to fulfill the request
- The Storm will issue a receipt of request, indicating a general timeline for final approval or denial
- Please note, an hourly appearance fee may be incurred. Fees may vary depending on the individual(s) and nature of the required appearance
